What CarePhrase is
CarePhrase is a care-note training coach for Healthcare Assistants, Support Workers, Care Assistants and Nursing Assistants working in UK care settings. It is a free, mobile-friendly training app that helps care workers communicate professionally and confidently — from writing care notes to speaking up in emergencies — by teaching them to observe clearly, record factually, and escalate appropriately.
Care workers write their own care notes and receive coaching feedback on clarity, professionalism, completeness and safeguarding awareness, along with a reflective question and scores to help them improve.
What CarePhrase is not
CarePhrase is not a care-note generator and not a care-note rewriter. It does not produce a finished care note for staff to copy and paste, and it does not rewrite the care worker's sentences. The app is a coaching and learning tool: staff must always write their own care notes in their own words. CarePhrase only provides guidance, identifies missing detail in plain English, and asks reflective questions so the care worker can improve their own writing.

Getting started
- Open the care-note training coach.
- Enter a first name or initials so your practice can be tracked (no personal data about service users is ever required).
- Choose a scenario from the library, or use free practice.
- Write your own note across the four sections — what happened, how it happened, what you did, and whether escalation or safeguarding action was needed.
- Receive coaching feedback and a reflective question on each section, then review your scores.
Worked example
This shows how CarePhrase coaches — it gives feedback, it does not write the note for you.
A carer writes: "Mary refused food and was angry."
CarePhrase coaches (it does not rewrite):
- Did well: You recorded that there was a change worth following up.
- Missing: Add the time, what food was offered, what the service user actually said or did, and who you informed.
- Language: "Angry" is a judgement — describe what you observed instead.
- Reflective question: Have you recorded only what you observed, without assuming how the person felt?
The carer then improves their own note using this feedback. CarePhrase never supplies a finished sentence to copy.
For supervisors
CarePhrase includes a supervisor view that lets a senior carer or manager see completed practice sessions — carer name, date, scenario, difficulty and overall score — and highlights any session where safeguarding awareness scored low so it can be followed up. The supervisor view is reached from the training tool and is protected by a PIN. The built-in PIN is for demonstration only and must be changed in the code before any real-world use; it is not secure production authentication.
Privacy and training disclaimer
Privacy — please read before you begin: Do not enter real service-user names, NHS numbers, dates of birth, addresses, phone numbers or any confidential personal information. Use a fictional name or write "the service user". A privacy reminder is shown at the point of entry on every screen so this is clear before any text is typed.
Data: CarePhrase does not require or store confidential service-user data. Practice progress is held in your browser's storage for your own use; if storage is unavailable it is kept only for the current session.
Training only: CarePhrase supports learning and reflective practice. It is not legally certified or CQC-approved and is not a replacement for employer policy, clinical judgement, safeguarding procedures or professional supervision. Staff must write their own care notes and follow employer policy, care plans, safeguarding procedures and local reporting requirements.
Accessibility
CarePhrase aims to be usable by everyone, including care workers using assistive technology. The app uses semantic HTML headings and landmarks, labelled form fields and buttons, keyboard-operable controls, a responsive mobile-first layout, and a light and dark theme that respects your device setting. We are working towards WCAG 2.1 AA. If you find an accessibility barrier, please contact us using the details below and we will do our best to put it right.
